Personal Care Assistance
Assisting patients with bathing, grooming, dressing, feeding, mobility, transfers (such as moving from bed to chair), monitoring skin health, and other daily care needs.
Vital Signs Monitoring
Measuring blood pressure, temperature, pulse, and respiratory rate, and documenting and reporting any irregularities or changes in vital signs to the supervising RN.
Basic Medical Procedures
Monitoring and reporting any changes in a patient’s health or behavior, providing medication reminders (without administering medications unless supervised), and assisting with respiratory care such as inhaler use or oxygen therapy.
Infection Control
Adhering to infection prevention procedures — particularly when caring for immunocompromised or bedridden patients—including proper hand hygiene, correct use of personal protective equipment (PPE), and safe disposal of medical waste such as needles and gloves.
Patient Mobility and Transfer Techniques
Practicing safe lifting methods and using assistive devices such as walkers, wheelchairs, and transfer belts to prevent falls and injuries. Assisting patients with walking, transferring from bed to chair, entering and exiting vehicles, and ensuring their comfort during positioning and transfers.
Documentation and Communication
Precisely recording patient care activities, observations, and any changes in condition. Collaborating with supervisors and other healthcare team members to address patient needs and progress, while applying techniques for managing chronic conditions, supporting individuals with dementia or cognitive impairments, and providing compassionate, sensitive end-of-life care.
